Nurse Practitioner

Department: East Dayton Health Center
Location: Dayton, OH

The Nurse Practitioner provides quality professional medical services to an especially diverse population with special needs in a primary care setting and limited by scope of practice.  

Duties:

  1. Provides care to patients:
    1. Obtains and documents medical history and physical exam.
    2. Orders labs and procedures.
    3. Makes referrals appropriately.
    4. Manages medical emergencies.
    5. Triages phone calls.
    6. Evaluates efficacy of treatment.
    7. Documents accurately and appropriately, including phone calls.
    8. Formulates assessment and plan.
    9. Understands and recommends appropriate community resources.
    10. Administers injections and medications.
  1. Advocates for patients.
  2. Collaborates with designated physician(s).
  3. Able to function independently.
  4. Able to work flexible hours.
  5. On call coverage optional depending upon level of training and functioning.
  6. Participates in at least 2/3 of staff conferences.
  7. Participates in training.
  8. Advises about health and prevention.
  9. Provides patient education
  10. Participates in QA activities.
  11. Participates in data interpretation and integration.

QUALIFICATIONS:

  1. Current State of Ohio RN license, MSN required
  2. Completion of an approved Nurse Practitioner program, family practice preferred. 
  3. 2 years experience providing primary care.
  4. Prescriptive Authority required
  5. Board Certification.
  6. Maintain BLS certification.
